New Patient Paperwork

Welcome to MK Dental Studio! We’re excited to meet you and look forward to caring for your smile.

To make your first visit as smooth as possible, we would appreciate you completing your new patient paperwork at least 24 hours before your appointment. This allows our team time to review your health history, insurance information, and dental concerns ahead of your visit so we can better prepare for your care.

Your forms will be texted to you several days before your first visit.

What to Bring to Your First Visit

Photo ID
Dental insurance card, if applicable
List of current medications
Any recent dental X-rays, if available.
